How to request an appointment

Lapal now offers a triage service to ensure people are receiving care from the appropriate health provider that is individual to their needs.

Urgent Appointments

Requesting an appointment that cannot wait for the next available routine appointment:

After submitting the form this will be triaged by a clinician and you will be contacted within 24 hours. You may be sent a text asking for further details so please fill in as much information as you can. This is to ensure we are booking you with the appropriate clinician and to determine if you need a telephone or face to face appointment. Any information provided is to increase the efficiency of the service.

For patients who are affected by digital exclusion we still offer the following services:

Ringing the surgery 0121 422 2345

The reception member will go through the form on the call for you and fill it in on your behalf. After submitting this form it will be triaged by a clinician and you will be contacted within 24 hours.

Attending the surgery

You will be asked to fill in a form and hand back to reception or reception will complete the digital form on your behalf. After submitting this form this will be triaged by a clinician and you will be contacted within 24 hours.

The reception team will be able to offer assistance for people who need it. Please be aware the receptionist will need to ask questions to gather information for the GP and to make sure you benefit from the appropriate health provider.

Not all appointments will require an appointment at the practice or when our appointments have been utilised for the day you may be offered an alternative appointment at the Extended Access Hub at Brierley Hill Health and Social Care Centre. We will be able to book this for you via our computer system but you will need to attend this alternative venue

Routine Appointments

If you need to book a routine follow up appointment with a particular GP, you can make an appointment up to four weeks in advance in the following ways:

Extended Out Of Hours Appointments

If you need access to an evening or weekend non-urgent appointment, please speak to the receptionists at the practice during normal opening hours. Your appointment may be with another practice in the Halesowen area and may not be your registered practice. In the Halesowen area the following practice are working in partnership to provide additional GP appointments.

Home Visits

Home visits are only for patients who are housebound or too ill to attend the surgery, as facilities here are far better for examination and treatment.

If you feel a home visit is needed, please phone 0121 422 2345 as early as possible, preferably before 11:00.

If a visit is required for a patient who requires support, it is most important, if at all possible, that their support person or relative should be present.

Cancelling an Appointment

If you are unable to keep your appointment, please give us as much notice as possible so that your appointment can be offered to someone else.

Fill in our Cancel Appointment Form below or telephone us on 0121 422 2345.

You can also reply CANCEL to your text message reminder as soon as possible, and at least 30 minutes before your appointment time. This will enable us to offer your appointment to someone else.
